LIABILITY RELEASE WAIVER

Goodspeed Agility | A DBA of Kidz Night Out LLC

Effective Date: May 1, 2026

Please read carefully.

In consideration of my child's participation in programs offered by Goodspeed Agility, a DBA of Kidz Night Out LLC ("the Company"), I, the undersigned parent or legal guardian, agree to the following:

Acknowledgment of Risk

I understand that speed and agility training, sport-specific training, and strength training involve inherent physical risks, including but not limited to sprains, strains, fractures, and other injuries. I voluntarily accept these risks on behalf of my child.

Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge Goodspeed Agility, Kidz Night Out LLC, their owners, coaches, employees, volunteers, and agents from any and all claims, damages, or liability arising from my child's participation in any program, including claims resulting from negligence.

Medical Authorization

In the event of an emergency, I authorize Goodspeed Agility staff to seek medical attention for my child if I cannot be reached. I accept financial responsibility for any medical costs incurred.

Indemnification

I agree to indemnify and hold harmless Goodspeed Agility and Kidz Night Out LLC from any claims, costs, or damages arising out of my child's participation, including any injury my child may cause to another participant.

Assumption of Risk

I acknowledge that I have had the opportunity to ask questions about my child's participation and that I understand the nature of the activities involved. I voluntarily assume all risk of injury or harm.

Severability

If any provision of this release is found to be unenforceable, the remaining provisions shall remain in full effect.

Governing Law

This release is governed by the laws of the State of California.

By accepting this release during online registration, I confirm that I am the parent or legal guardian of the registered athlete, that I am at least 18 years of age, and that I have read and understood this document in its entirety.

This consent is valid indefinitely unless otherwise rescinded in writing by the parent/guardian of the participant.
